Holbrook Healthcare Center in Buckhannon, WV seeks a Nurse Practitioner (Adult and/or Gerontology) to deliver high-quality primary care in a high-touch model. The role supports seniors across SNF, assisted living, and telehealth settings, with collaboration from a physician and care team. The NP will diagnose and manage chronic and acute conditions, provide education, and ensure compliant documentation in the EMR.
