Registered Nurse - Case Management - Hospital

Job Description

Job Summary:
Provide specialized case management services to hospitalized patients, coordinating care plans and ensuring optimal patient outcomes. Play a pivotal role in adapting care strategies to meet individual patient needs during hospitalization.

Responsibilities:

Assess patients' clinical needs and develop comprehensive care plans.
Collaborate with multidisciplinary teams to coordinate patient care and discharge planning.
Monitor and evaluate patient progress, adjusting care plans accordingly.
Educate patients and their families on post-discharge care and support.
Document clinical interventions and patient responses accurately in electronic medical records.
Comply with hospital policies, procedures, and regulatory standards to ensure quality care.

Qualifications:

Hold a valid RN license with at least 2 years of acute care or hospital experience.
Demonstrate proficiency with electronic medical records systems (Meditech and Midas preferred).
Exhibit strong clinical assessment and critical thinking skills.
Possess excellent communication and interpersonal skills for effective team collaboration.
Be willing to work alternating weekends, holidays, and flexible schedules as required.
Prior case management experience in a hospital setting is preferred.
