Under the direction of a licensed Physical Therapist, the PTA will provide skilled therapy services designed to help residents improve mobility, strength, function, and independence.Key ResponsibilitiesProvide physical therapy treatments as directed by the supervising Physical TherapistImplement individualized treatment plans based on physician orders and therapy goalsMonitor resident progress and communicate changes to the Physical TherapistDocument treatment sessions, resident responses, and progress notes accurately and timelyAssist with discharge planning and caregiver educationCollaborate with nursing staff and interdisciplinary team members to achieve positive resident outcomesParticipate in care conferences and team meetings as neededMaintain a clean, safe, and organized therapy environmentEnsure compliance with all state, federal, and facility regulationsSupport residents in achieving their highest level of function and independenceQualificationsEducation & ExperienceGraduate of an accredited Physical Therapist Assistant programExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) and Casamba preferredStrong documentation, communication, and organizational skillsLicenses & CertificationsCurrent South Carolina Physical Therapist Assistant (PTA) license in good standingMust maintain all required continuing education requirementsMust remain in good standing with the State BoardWhat Makes You a Great FitCompassion for an underserved populationStrong clinical and interpersonal skillsPositive attitude and team-first mindsetDependable, flexible, and self-motivatedPassion for helping residents achieve their highest level of independenceApply TodayJoin a therapy team dedicated to helping residents regain strength, mobility, and confidence.
