Licensed Practical Nurse (LPN) - LPN

LPN - Home Health

Schedule: 8-hour shifts

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 hours per week

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years' experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
• Knowledge of electronic medical charting and time-keeping software (Homecare Homebase)
• Proof of current Hepatitis consent/declination and TB vaccination record

Roles & Responsibilities:

• Administer skilled nursing care to patients in their residences in accordance with the established plan of care.
• Assist in providing nursing services and identifying patients' physical, psycho-social, and environmental needs.
• Participate in planning, coordinating, and implementing care in conjunction with the supervising RN.
• Effectively communicate with patients/families, co-workers, and other health care professionals.
• Educate patients/families on appropriate self-care techniques and assess their response to care.
• Utilize infection control measures to protect both staff and patients.

Compliance / Notes:

• Must have good and regular attendance.
• Participate in annual in-service and performance improvement programs.
• Must read, write, and speak fluent English.
