Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active California Physical Therapist license required to start the assignment.
• BLS Certification required from AHA or ARC.
• Home health experience required.
• HCHB (Homecare HomeBase) experience required.
• Productivity familiarity: ability to meet productivity expectations (example: approximately 6 points per day via combinations of follow-up and Start of Care visits).
• Ability to provide services in the Woodland Hills and Encino territory (91302 and 91316) and manage travel within that territory.
• Reliable transportation and ability to perform home visits in a community setting.

Assignment Details

• Facility type: Home Health.
• Shift: Days.
• Shift details: 8-hour shifts, Monday through Friday.
• Schedule: Monday to Friday, approximately 8:00 AM to 5:00 PM.
• Positions available: 1.
• On-call or weekend requirements: None.
• Floating: Candidate may be required to cover the local territory (Woodland Hills and Encino) as needed.
• Documentation system required: HCHB experience.
