Social Worker (Program Coordinator)

Allied Health Professional DutiesDuties for the position include but are not limited to the following:In conjunction with HCHV and HUD-VASH Leadership, create, improve, and maintain systems to ensure intake and assessment is completed in a timely manner, offering Veterans the most appropriate referrals and supports.Co-facilitating weekly clinical care coordination meetings with the HUD-VASH Team.Outreach to and development of partnerships with community partners including landlords, group homes, and other partners able to assist Veterans with long-term housingAssisting with program improvement projects, and program development to meet Directives and other guidance specific to the HCHV and HUD-VASH Programs.Clinical consultation and guidance for GS9/11 Social Workers in HUD-VASH.Providing ongoing education and training for GS9/11 Social Workers specific to case management practices and expectations, and treatment planning.Monitoring HOMES data and Homeless Programs Office reports such as the By Name List and communicating with the team and leadership about progress and areas for improvement.Assist with coordination of Homeless Programs events such as Outreach Surges and Stand Downs.Develops and provides education to internal and external partners specific to VHA servicesConducts assessments within the HCHV Assessment Clinic via in-person, phone and video connect according to the Veteran's needs and abilities.Responds to Consults and Homeless Hotline calls by conducting assessments and connecting Veterans to appropriate services.Conducts outreach, attending community events, and seeking to identify Veterans who may benefit from HCHV and HUD-VASH support services.Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.Assisting Veterans with completion of paperwork and processes related to securing housing and/or other supportive services.Utilizing Housing First and other evidence-based practice models to promote Veteran engagement and self-efficacy.Assisting HUD-VASH ineligible Veterans with housing placement, which may involve conducting independent housing searches, and working with Veterans and community services to establish appropriate support.Advocating on behalf of Veterans and their families with landlords, community providers and internal VA stakeholders.The position requires the ability to safely and legally operate a government vehicle.Work Schedule: Monday through Friday, 7:30 a.m. - 4:00 p.m.Recruitment/Relocation Incentives (Sign-on Bonus): Not AuthorizedPermanent Change of Station (Relocation Assistance): Not AuthorizedPay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Telework: Ad-hocVirtual: This is not a virtual position.Functional Statement #: 000000Permanent Change of Station (PCS): Not Authorized
