Our organization is excited to announce an opportunity in the Sub-Acute and Long-Term Care sector, which is rapidly growing and offers excellent work-life balance (no on-call duties or weekend shifts)!

We are looking for a full-time provider (40 hours per week) for our facility located in Santa Fe, NM. This role is open to either a nurse practitioner or a physician assistant, and we encourage new graduates to apply.

The salary for this position ranges from $115,000 to $125,000 annually, along with an incentive compensation bonus program that has no cap. About Us

Key Responsibilities

The Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaborating on-site with the Center Leadership Team to meet specific center goals and priorities. Additionally, the Advanced Practice Provider will work alongside the Medical Director and/or Attending Physician to deliver patient-specific medical care.

The Advanced Practice Provider will offer primary and urgent care services to residents of the nursing center, working in conjunction with other medical professionals at the facility. This position is integral to achieving our mission of enhancing lives through high-quality healthcare and compassionate service.

Qualifications

To qualify for this role, the Advanced Practice Provider must have graduated from an accredited nursing program, hold a registered nurse license, and be certified as a Nurse Practitioner with a Master's Degree in Nursing, focusing on Older Adult or Family Nurse Practitioner (FNP) specialties. Other concentrations may be considered based on experience and organizational needs. For Physician Assistants, graduation from an ARC-PA accredited program is required, along with passing the PA National Certifying Exam (PANCE) and obtaining state licensure. NCCPA certification is also necessary.

A minimum of two years of experience working with adult or geriatric populations is preferred.

In states where practice regulations permit, APRNs must hold an unrestricted professional license and DEA registration for each state of practice. Similarly, PAs must have an unrestricted professional license and DEA registration for their respective states.
