Job Description

Registered Nurse Case Manager – Hospice

Location: Mount Vernon, IL, USA Schedule: Monday–Friday, 8:00 AM–5:00 PM (daily start/end times may vary) On‑Call: 1–2 weekend shifts per month (8A–5P or 5P–8A depending on staffing) Holidays: 2 required per year (1 summer, 1 winter) + 3 floating holidays Travel: Local travel across multiple counties required

Overview

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client's hospice team and make a meaningful impact in the lives of patients and families. You will provide compassionate care, coordinate services, and serve as a clinical leader within a supportive, mission‑driven environment.

Key Responsibilities

Manage and oversee patient care in hospice settings
Coordinate care plans with interdisciplinary healthcare teams
Provide leadership, guidance, and support to nursing staff
Ensure timely and accurate documentation
Maintain compliance with hospice standards and regulatory requirements
Travel locally to patient homes across multiple counties

Required Qualifications

High School Diploma or GED
Valid Registered Nurse (RN) license in the state of Illinois (IL)
1+ year of Hospice experience
1+ year of Management or leadership experience
1+ year of experience in any of the following:
- Home Health Care
- Epic Home Health
- Home Health
Valid certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Must not have applied to or worked for Residential Healthcare Group within the last 6 months
Must be comfortable with required travel for this position

Preferred Qualifications

RN license number provided at time of application

Travel Requirements

Local travel required across multiple counties
Must have reliable transportation and willingness to travel daily
