An Official website of the Commonwealth of Massachusetts Job Description - Human Services Coordinator II, Case Management Supervisor, North Shore Site Office (260005YA) Job Description Description The Department of Mental Health is seeking qualified professionals for the position of Human Service Coordinator II to promote the recovery of DMH clients through the supervision of case managers and serving as a resource to DMH funded programs. The planning and implementation of well-coordinated services and benefits based on the clients' preferences, strengths, and needs and on the availability of resources is a responsibility of this position along with ongoing assessment of risk and attention to risk mitigation. Human Service Coordinator II may be responsible for a reduced case load with supervisory responsibilities. In addition, the position is responsible for performing Needs & Means assessments of assigned individuals who are determined eligible for DMH services. This is a full-time, 37.5 hours per week position and the incumbent will work a schedule from 9:00 a.m. –5:00 p.m. with Saturday and Sunday as days off. Duties and Responsibilities (these are a general summary and not all inclusive) Provides weekly supervision to case managers to ensure that quality services are provided to clients. Participates in the Case Management Record Review Committee and follows‑up on any observed trends in deficiencies in client records or MHIS with assigned case managers. Provides direct care by conducting needs assessments of clients eligible for DMH services to determine the type and intensity of services needed. Responsible for a reduced caseload. Provides supportive counseling to clients and/or significant others to identify, accept, and secure services. Develop and maintain working relationships with community agencies and other state programs and agencies to identify unmet needs and to effectively access a comprehensive range of services for clients. Demonstrates insight and creativity in the identification and development of new programs to effectively address specific client needs which are currently not being met by existing services. Assumes and/or delegates responsibility for supervises' consumers when supervisee is absent or unavailable, assuring continuity of services to clients. Assists clients in developing and maintaining a supportive network in the community, particularly in relation to family and friends for clients to develop and maintain meaningful social relationships. Assesses client risk issues, presents at Clinical Reviews, and assures that case managers follow through with all treatment/risk reduction recommendations. Monitors assigned programs by meeting with vendor staff monthly. Meets monthly with assigned programs to review clients' utilization of identified services and to review whether clients continue to need the level of service provided. Required Qualifications Considerable ability to deal effectively with clients and community agencies. Ability to accurately conduct needs assessments and to develop and implement an individual service plan. Skills in making productive referrals, negotiating assertively from an advocacy‑based client centered perspective with a variety of public service agency professionals. Ability to work non‑judgmentally with clients whose belief system may be predicated on values incongruent with those of the Case Manager Supervisor. Ability to supervise and evaluate performance of staff. Ability to respond to multiple demands by effectively prioritizing tasks. Preferred Qualifications Strong and effective interpersonal skills, including the ability to listen well, relate to others and demonstrate sensitivity and respect for individuals. Demonstrated commitment to serving a diverse population with cultural competence and sensitivity, as well as the ability to work with a wide range of constituencies with diplomacy and tact. Proficiency in Microsoft Office applications including Outlook, Word, and Excel. Qualifications Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required. In addition applicants must have at least four (4) years of full-time, or equivalent part-time, case management of clinical/therapeutic experience. Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment. Salary placement is determined by a combination of factors, including the candidate’s years of directly related experience and education, and alignment with our internal compensation structure as set forth by the Human Resources Division’s Hiring Guidelines.
