Social Worker/Case Manager Raleigh, United States | Posted on 01/03/2025 Location: Various locations across North Carolina The Social Worker/Case Manager supports individuals and families in accessingresources and services to address their social, emotional, and economicchallenges. This role involves assessing client needs, creating care plans, and advocating for client well-being. KeyResponsibilities: Conduct assessments toidentify client needs, challenges, and goals. Develop individualizedservice or care plans. Provide counseling,emotional support, and crisis intervention. Connect clients withcommunity resources such as housing, healthcare, and education. Advocate for clients in securingnecessary services and benefits. Monitor and document clientprogress and update plans as necessary. Maintain detailed andconfidential client records. Collaborate with otherprofessionals and agencies to provide comprehensive support. Requirements Bachelor’s degree in socialwork, psychology, or a related field (Master’s preferred). Minimum of 2 years ofexperience in case management or social work. Strong understanding ofsocial service systems and community resources. Excellent organizational andinterpersonal skills. Proficiency in casemanagement software and tools. Empathy, culturalcompetence, and ability to handle sensitive situations. #J-18808-Ljbffr
