Hospice Registered Nurse Case Manager

Transitions Hospice in Illinois is seeking an RN Case Manager to lead clinical care for patients at home, coordinating the interdisciplinary team and guiding care plans. You will assess needs, administer orders, and ensure accurate documentation while supporting families in challenging times. Active RN license in the state of employment and hospice/home health experience are preferred.
