Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and tailor individualized treatment plans to meet their needs
• Administer targeted physical therapy interventions focusing on mobility, strength, balance, and overall functionality
• Work closely with nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
• Provide education to patients and their families about therapeutic objectives, discharge planning, and at-home exercises
• Ensure proper documentation of evaluations, treatments, and patient progress adhering to facility standards and regulations
• Engage in interdisciplinary meetings and care team rounds to guarantee effective collaboration in patient care

Education

• Doctorate in Physical Therapy (DPT) or another accredited equivalent degree

Certifications

• State licensure as a Physical Therapist
• Preferred: CPR certification
