Part-Time Nurse Practitioner (NP) or Physician Assistant (PA) - Endocrinology Schedule: Part-Time | Monday-Friday | Flexible Hours | No Call | No Weekends Vital HealthCare Solutions is hiring a Nurse Practitioner (NP) or Physician Assistant (PA) with endocrinology experience to provide care in long-term care and skilled nursing facilities . This part-time position offers schedule flexibility, clinical autonomy, and a collaborative team environment focused on delivering high-quality care to geriatric patients. Position Summary The NP or PA will provide comprehensive endocrine care, including diabetes management, thyroid management, and medication management, for patients in long-term care settings. Key Responsibilities
- Diagnose and manage diabetes mellitus (Type 1 and Type 2)
- Prescribe and adjust insulin and non-insulin diabetic medications
- Manage and interpret Continuous Glucose Monitoring (CGM) systems
- Provide thyroid disorder management (hypothyroidism, hyperthyroidism)
- Perform patient assessments, treatment planning, and follow-up care
- Collaborate with interdisciplinary teams in long-term care facilities
- Maintain accurate and timely electronic medical record (EMR) documentation
- Active and unrestricted Pennsylvania NP or PA license
- Experience in endocrinology, diabetes management, or internal medicine
- Knowledge of insulin therapy, CGM devices, and thyroid medications
- Experience in long-term care, skilled nursing, or geriatric care preferred
- Strong clinical assessment and diagnostic skills
