Hospice Registered Nurse (RN) – Part-Time / Per Diem At AdvisaCare, we believe hospice care is more than a service—it is a sacred calling to provide presence, compassion, and dignity at life’s most delicate intersections. We are looking for a dedicated RN to join our Portage-based team, bringing expert clinical care and a warm heart directly to patients and families in the comfort of their own homes. Your Impact In this role, you step away from the chaos of hospital corridors to focus on what truly matters: the patient. Your expertise will provide the comfort and peace of mind that allows our clients to live their final days with grace. You will: Provide personalized, one-on-one clinical care that honors each patient’s unique journey. Act as a vital pillar of support for families, offering education and emotional reassurance. Collaborate with a supportive interdisciplinary team during regular group meetings to refine plans of care. Ensure continuity of care through focused supervisory visits and clinical observation. About You We are searching for a professional who views hospice not just as a job, but as a commitment to excellence and empathy. The ideal candidate brings: A current RN license and a deep-seated belief in the value of end-of-life care. Previous experience in a hospice or home health setting. Exceptional organizational skills and the ability to work efficiently and independently. The flexibility to support our rotating on-call schedule (5 PM - 8 AM), ensuring our families are never alone when they need us most. Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs Implement the individualized plan of care and revisions as needed Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern Supervision of paraprofessionals providing services to patient Preparing clinical and progress notes, participating in IDG conferences Participate in Hospice performance improvement program Maintain high level of customer satisfaction with our clients, families and caregivers Communication of information using current process and technology within organization Requirements: Current Michigan License in good standing Minimum 1 year hospice experience Empathetic approach to patients and caregivers Participate in field visits daily, weekly office meetings as indicated Ability to adapt to patient’s status and needs on a full time on call basis Rewards AdvisaCare Can Offer YOU! 401 K Retirement Plan Medical Benefits Effective NOW!
