Part Time PRN - Home Health Physical Therapist

Position Overview A Physical Therapist (PT) is sought to assess, teach, and deliver therapy interventions based on physician directives within a home health setting. Responsibilities include conducting evaluations of mobility, strength, and function; developing and updating treatment plans; delivering therapy services; documenting progress; and collaborating with healthcare team members. The PT will supervise personnel such as PT Assistants and Home Health Aides, complete assessments like OASIS where permitted, and ensure compliance with policies, regulations, and documentation standards. Qualifications Must hold a degree in Physical Therapy from an accredited program, possess a current unrestricted state license, and demonstrate proficiency in clinical assessments and documentation.
