$60,000 - $75,000 - Annual Exempt

Reports To: Clinical Director or Case Management Supervisor

About This Opportunity

All Points North is seeking an experienced and compassionate Integrated Care Advisor – Case Manager to join our clinical care coordination team. In this role, you will serve as the primary case manager for our clients, orchestrating seamless, clinically appropriate, and financially transparent treatment experiences. You'll be the vital link connecting clinical teams, medical providers, financial services, and external stakeholders—ensuring every client receives coordinated, person-centered care from intake through discharge and beyond.

The ideal candidate can manage complex caseloads independently, apply sound clinical judgment, and collaborate effectively across our multidisciplinary team. This role is well suited for someone who brings experience in behavioral health case management and is ready to take ownership of client advocacy, care coordination, and continuum-of-care transitions.

You'll work in a supportive, evidence-based treatment environment where your coordination efforts directly impact client outcomes. This position offers the opportunity to develop expertise in utilization review, financial counseling, and integrated care delivery while working alongside dedicated professionals committed to transforming lives.

What You'll Do
  • Independently manage the full client lifecycle—from intake and onboarding through discharge planning and aftercare coordination
  • Conduct comprehensive client intakes, orientation, and documentation within established timelines, ensuring accuracy in all EMR entries
  • Maintain consistent daily interaction with clients to monitor progress, assess psychosocial and medical needs, and provide advocacy in treatment planning
  • Develop and implement individualized aftercare action plans and discharge strategies that ensure continuity of care
  • Coordinate with clinical, medical, utilization review, admissions, and finance teams to align treatment recommendations with clinical best practices and insurance requirements
  • Facilitate financial conversations with clients and families regarding cost of care, insurance coverage, financial responsibility, and treatment implications
  • Assess and recommend appropriate levels of care throughout treatment, supporting medical necessity determinations and facilitating step-down transitions
  • Identify concerns, resolve routine coordination issues, and escalate higher-risk or complex matters to clinical leadership appropriately
  • Communicate treatment extensions, denials, and level-of-care changes to clients, families, and external stakeholders with clarity and compassion
  • Coordinate with external partners including attorneys, probation officers, referral sources, and community providers to support legal compliance and continuity of care
  • Maintain timely, accurate, and complete documentation in Kipu EMR and other required systems
  • Attend all required meetings—including staff huddles, utilization review, aftercare coordination, and extended care discussions
  • Support the development and improvement of case management processes and workflows
  • Participate in department training, professional development, and cross-functional initiatives
  • Maintain compliance with APN policies, professional standards, and applicable legal and regulatory requirements
  • Perform other duties as assigned to support the team and APN operations

  • Demonstrates strong advocacy for clients while balancing clinical, financial, and operational considerations
  • Contributes practical ideas that strengthen care quality, coordination efficiency, and the client experience
  • Receives positive feedback from clients, families, and interdisciplinary team members
What You'll Bring Required Qualifications
  • Bachelor's degree in behavioral health, psychology, social work, counseling, or related clinical field
  • Minimum 2+ years of behavioral health and client care experience, preferably in case management, care coordination, or utilization review
  • Demonstrated ability to manage complex responsibilities independently and prioritize competing needs
  • Strong written and verbal communication skills with the ability to communicate clearly with diverse audiences
  • Sound judgment and the ability to recognize when an issue requires escalation or clinical consultation
  • Working knowledge of behavioral health levels of care (RTC, PHP, IOP, OP) and continuum-of-care transitions
  • Understanding of utilization review, medical necessity determination, and insurance authorization processes
  • Demonstrated ability to facilitate financial conversations with clients and families
  • Proficiency in Microsoft Office and web-based systems; ability to learn new software quickly
  • Valid driver's license with acceptable driving record
  • Ability to pass background check, urine drug screen, and criminal records clearance
  • High moral and ethical values; commitment to confidentiality and appropriate professional boundaries
Preferred Qualifications
  • Master's degree or current graduate student status in a clinical field (MSW, MA, MPA, or related discipline)
  • Experience with Kipu EMR system or similar behavioral health platforms
  • Experience with utilization review and insurance authorization processes
  • Knowledge of NAADAC ethical practice standards or similar professional guidelines
  • Experience facilitating level-of-care transitions and step-down placements
  • Experience supporting or mentoring newer team members
  • Bilingual English/Spanish skills

Team and Partnerships

Key Partners: Clinical teams, medical providers, utilization review specialists, admissions staff, finance and billing teams, families, referral sources, external providers, legal entities, and community partners.

Team Support Responsibilities: You may provide guidance, training, or support to newer case management team members and contribute to the development of department processes and best practices.
