We are seeking a compassionate and dedicated Social Worker (LSW/CSW) to join our interdisciplinary team. In this role, you'll help residents and their families navigate their healthcare journey by providing emotional support, advocacy, and care planning that enhances each resident's quality of life. Social Worker (LSW/CSW) Qualifications & Responsibilities: Must possess a current New Jersey LSW or CSW license Previous sub‑acute and/or long‑term care experience preferred Complete resident social histories and psychosocial assessments Identify residents' psychosocial, emotional, and mental health needs Collaborate with the interdisciplinary team to support resident‑centered care and protect resident rights Recognize, prevent, and address resident abuse in accordance with applicable laws and professional standards Maintain accurate documentation and resident records Experience with PointClickCare electronic medical records preferred Build positive relationships with residents, families, and team members Ability to work independently and collaboratively Perform other duties as assigned by the Director of Social Services and Administrator Why Join Complete Care at Willow Creek?
