Department Specific Requirements
If assigned to Home Infusion: requires one year of home infusion pharmacy experience and/or one year of hospital based clinical pharmacist experience. Prefer completion of a residency program in home infusion pharmacy.

If assigned as the Emergency Medicine Pharmacist Specialist: requires one year of Emergency Department pharmacy experience and/or one year of hospital based clinical pharmacist experience in the critical care setting. Prefer completion of a pharmacy residency program in Emergency Medicine.

If assigned as the Infectious Disease Specialist: requires one year of hospital based clinical pharmacist experience. Prefer completion of an a pharmacy residency program in Infectious Diseases.

If assigned as the Anemia Management Specialist: requires one year of hospital based clinical pharmacist experience. Hospital or Ambulatory Care based anemia management experience is preferred.

If assigned as the Outpatient Oncology Pharmacist: Completion of PGY 1 residency or at least two years of acute inpatient hospital pharmacist experience. Prefer completion of pharmacy residency in Oncology

OFF CAMPUS:

Performs all the principal duties of a Pharmacist-In-Charge. Establish policies and procedure for the department. Reviews and revises policies and procedures at least once a year. Participates in coordination of principal department operations, including committee functions. Serves as a member of the Pharmacy and Therapeutics Committee, and advisory committee to the Medical Staff. Interacts with other department heads to solve mutual problems. Guides, directs, evaluates, and trains pharmacy staff and assumes responsibility for quantity and quality of their work. Completes staff scheduling. Assists Manager with performance evaluations for assigned employees. Participates in pharmacy operation and control management. Provides inventory control, periodic performance activity reports to Pharmacy Director, assists in budget activities. Sets and reviews annual goals with manager.

MEDICAL CENTER CAMPUS:

Assists the Inpatient Pharmacy Manager in organizing and directing, safe, quality, customer-oriented and efficient pharmacy daily operations. Monitors and supervises the daily workflow of all pharmacy staff on duty to ensure optimal workflow and timely delivery of medications. Available as a resource for staff to guide and/or support day-to-day issue resolution. Stays abreast of applicable federal and state laws as professional standards of practice. Assists Inpatient Pharmacy Manager in monitoring and assuring ongoing compliance. Supports the regulatory/accreditation survey process by ensuring ongoing survey readiness, participating in survey activities such as interviews or tracers, and; assisting and/or facilitating additional department efforts to achieve desired outcomes. Assesses operational performance and implements means to improve inpatient pharmacy operations including pharmacy systems and procedures. Continuously evaluates existing procedures for ways of improving quality of service to customers and assuring procedures are in line with practice and regulatory standards. Assures safe medication use for patients by optimizing applicable automation technologies in the medication distribution process. Oversees the monitoring of all pharmacy related logs for compliance. Supports the development and completion of compounding quality assurance reports; following up as appropriate when indicator findings are outside of defined thresholds. Actively contributes to the planning for and achievement of annual departmental objectives by coordinating, completing and/or assisting others to implement identified tactics that meet the objective. Supports the pharmacy's pharmacoeconomic efforts by identifying and resolving operational inefficiencies. Coordinates assignment of special projects related to pharmacy operations. Oversees Controlled Substance Safety Inventory Management and Controlled Substance Safe Technicians to assure complete and accurate documentation and timely follow up of controlled substance discrepancies. Communicates and coordinates with the Inpatient Pharmacy Manager on issues pertaining to Human Resources to support evaluation of staff performance. Establishes, maintains and coordinates holiday, sick leave and work schedule of pharmacist staff. Responsible for assuring coverage or staffing of shifts when employees are absent or openings exist. Performs all the duties of a pharmacist and rotates through shift coverage as required. Facilitates pharmacy verbal and written communication. Assists with special projects as needed. Participates and attends various hospital committees as appropriate. Able to adapt to change productively and perform other duties as assigned. Assists with operational orientation, training, competency assessment, education, and communication. Sets and reviews annual goals with manager.

EMPLOYEE PHARMACY:

Performs all the principal duties of a Pharmacist-In-Charge. Adheres to all Federal and State Laws pertaining to dispensing, filling, of authorized prescription medications and supplies, including but not limited to receiving and verification of prescriptions, selection of medication and filling prescriptions. Dispenses medication, supplies or equipment accurately and efficiently. Compounds medications accurately and efficiently. Maintains 100% accuracy in measurements and dose calculations. Maintains proper documentation and logs for compounding. Knowledgeable, compliant and adherence to regulations in dispensing control and non-controlled substances, including proper record keeping and documentation. Provides direction to technical staff and support staff. Guides, directs, and trains pharmacy technicians and clerks. Assumes responsibility for quantity and quality of their work. Assists Employee Pharmacy customers with insurance issues, including obtaining prior authorizations, contact physicians or other health providers on possible therapeutic interchanges. Maintains knowledge and facilitates third party insurance, including Prior Authorization process, Medication Formularies, Wellness programs, coupons, discounts, workers compensation and other sources of payments, member's enrollment and eligibility. Provides consultation services regarding medication utilization and safety as appropriate for both prescription and over the counter medications and in guidelines of the California State Board of Pharmacy. Consults with other health care providers, information resources and payers to optimize therapy and maximize safety. Provides cognitive services for disease management and wellness programs. Programs include, but not limited to assessments, care planning, monitoring, including labs evaluation, adverse drug reactions and compliance. Provides in-service training to other healthcare providers, staff and public when appropriate. Maintains records and reporting in accordance to California State Board of Pharmacy prescription records, electronic records and faxes. Reports, corrects and maintains CURES information, Control Substances log and prescription records, Employee Pharmacy specific records such as prescription logs, sales transactions, monitoring receipts of payments including reconciliation, sales taxes. Provides weekly report on deposit record and sales transaction reports. Assists in maintaining the data bases of all systems used by the Employee Pharmacy, including updates to the systems, trouble shooting, contacting the appropriate helpdesks with issues to resolution. In conjunction with pharmacy administration, determines adequate inventory to process prescriptions in a timely fiscally responsible manner. Utilizes cycle counting and other inventory management tools and strategies. Responsible for annual physical inventory. Assists pharmacy administration in Process Improvement and Quality initiatives and processes. Reports any dispensing, medication errors or HIPAA violations in accordance to organization policies.

OUTPATIENT ONCOLOGY PHARMACIST AT INFUSION CENTER:

Assists pharmacy leadership in organizing and directing daily pharmacy operations and pharmacy personnel, including training, education, and competency completion to ensure regulatory requirements are met. Assists pharmacy leadership in maintaining records for pharmacy licenses, staff licenses, certifications and competencies as appropriate. Participates in coordination of principal pharmacy related department operations, including committee functions participating in medical staff and organizational related committee meetings as they relate to pharmacy services in outpatient oncology. Interacts with other department leadership to solve mutual problems. Guides, directs, evaluates, and trains pharmacy personnel and assumes responsibility for quantity and quality of their work. Completes staff scheduling as it relates to pharmacy services. Assists Manager with performance evaluations for assigned pharmacy employees. Assists in drug inventory management and provides periodic performance activity reports to Pharmacy Director and assists in budget oversight as it relates to pharmacy services and in collaboration with clinic leadership. Establishes annual goals with manager and is able to successfully show progressive achievement. Oversees compounded preparations within the outpatient oncology infusion center pharmacy and collaborates with key stakeholders to ensure quality of compounded preparations and compliance with regulatory and best practices involving compounding. Participates in regulatory body surveys, fielding questions and matters related to compounding, and other pharmacy related operations and services. Develop, implement and maintain pharmacy related oncology infusion center related policy, procedures, order sets, guidelines & operational workflows to ensure quality, safety, compliance, and operational performance. Reviews and revises pharmacy related policies and procedures at least once a year. Develops, implements, maintains and oversees all aspects of the Compounding Quality Assurance Program; ensuring education and training of pharmacy staff relative to the program, ensuring required documentation, and reporting at intervals set by regulations and department policy. Leads efforts in continuous process and quality improvement of the compounding and operational processes to maximize efficiency, safety, financial performance and ongoing survey readiness. Oversees medication procurement, inventory controls, shortage management and formulary processes within the outpatient oncology setting. Collaborates with Pharmacy Leadership to help ensure regulatory compliance with regards to the 340b program. Serves as preceptor for pharmacy residents and interns.

WHEN ASSIGNED TO WORK AS A CLINICAL PHARMACIST:

Dispenses medication, supplies or equipment orders accurately and efficiently. Follows written policies and procedures regarding medication administration. Consults with other health care providers and information resources to optimize therapy and maximize safety for medication administration for assigned areas. Uses PYXIS units on the nursing units. Responsible for answering all inquiries from physicians and other staff regarding medication administration issues. Compounds IV medication and solutions accurately and efficiently. Uses strict aseptic technique and maintains 100% accuracy in measurement and dose calculations when compounding. Complies with OSHA guidelines and procedures. Maintains absolute sterile technique in IV room at all times. Guides, directs, and trains pharmacy techs and assumes responsibility for quantity and quality of their work. Provides in-service training to other healthcare providers and staff when appropriate. Monitors patient's drug and nutritional therapies and maintains their clinical charts with complete and timely documentation. Provides patient specific dosing and monitoring information. Provides consultation and actively participates in patient treatment. Offers cost effective and safe alternatives when appropriate. If assigned to fill outpatient prescriptions, does so in accordance with federal and state law and in accordance with hospital policies Patients shall be counseled upon receipt of prescription on the appropriate use, storage and potential adverse reactions. Responds to Code Blue alert as a top priority. Makes up all dilutions, IV's, etc., ordered by the physician. Anticipates patient drug needs and responds accordingly. Fills all narcotic requisitions as presented by the techs after ascertaining correctness of requisition. Maintains all appropriate records demanded by federal and state laws and KH procedures. Conducts monthly nursing station inspections, checking for cleanliness, outdates, orderliness, and unauthorized drugs as required by The Joint Commission and Title 22 and keeps all appropriate records. Organizes and plans monitoring of drugs encompassed by pharmacokinetics following all department procedures. Consults with physician on optimal drug therapy. Monitors patients following department procedures. Performs antibiotic chart review following department procedure. Generates written reports. Provides information to modify therapies if required. Quarterly audit of single drug group. Prices patient charges the day they accrue under direction of manager.
