Your Role as a PT Deliver personalized in-home therapy sessions tailored to each patient’s needs and goals. Develop treatment plans that improve strength, balance, and mobility, reducing fall risks and promoting independence. Educate patients and families on safe movement techniques and daily living activities. Document patient progress and care using WellSky/Kinnser EMR. Collaborate closely with physicians, case managers, and other care providers to ensure seamless care. What We’re Looking For Licensed Physical Therapist in California (required). AHA CPR Certification. Minimum 1 year of experience. Home Health experience is a plus but not required. Reliable transportation, valid driver’s license, and auto insurance. Strong communication, organization, and problem-solving skills. Compassionate, personable, and self-motivated.
