Remote IRIS Care Consultant — Help Adults Thrive

Molina Healthcare

Molina Healthcare seeks an IRIS Consultant to support participants in the Wisconsin IRIS program. This remote role involves field visits to participants' homes and communities, coordinating care, resources, and budgets to meet long-term goals.

You will work closely with participants to navigate IRIS offerings, document progress, and collaborate with agency staff to ensure successful plan implementation and service delivery.
