Job Title: Physical Therapist - Home Health

Duration: 13 weeks

Shift: Day

Shift Hours per Shift: 1st 8 hours mileage in addition to rate

Experience: Minimum years of experience required

License: Physical Therapy licensure in all applicable states of practice

Certifications: CPR or BLS certification from approved vendor

Must-Have: Valid Driver's License and must be at least 21 years of age

Job Summary: Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions while meeting department objectives. Serves as co-leader in an interdisciplinary approach to patient groups.

Job Requirements:

• Doctor of Physical Therapy (DPT) degree required, or foreign equivalent if obtained after January 2016.
• Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
• Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
• Provides therapy interventions using appropriate treatment protocols and techniques.
• Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
• Completes comprehensive and accurate clinical documentation within established deadlines.
• Supervises assigned assistants.
• Performs other job-related duties as assigned.

• Commitment to the safety and well-being of associates and patients.
• Required vaccinations are a condition of employment, with medical and religious exemptions available.
