• Low daily patient volume

• Geriatric-focused practice

• 11-person interdisciplinary care team
Assignment Details \n
• Start: November 2026
\n
• End: Ongoing
\n
• Commitment: Minimum 2 weeks/month; full-time preferred
\n
• Schedule: Monday-Friday, 8am-5pm
\n
• 3-4 days/week also considered
\n
• Setting: Outpatient clinic + community-based visits
\n
• Population: Adults age 55+
\n
• Volume: ~4-6 patients/shift
\n
• Community visits may include assisted living, SNF and private residences
\n
• Guaranteed hours: 8/day
\n
• EMR: Epic
Clinical Details \n
• Family Medicine or Internal Medicine with geriatric experience
\n
• Combination of in-clinic and community-based patient care
\n
• Community visits typically supported by a team nurse
\n
• Care coordinated through an 11-person interdisciplinary PACE team
Requirements \n
• BC or truly BE in Family Medicine or Internal Medicine
\n
• Active OR license
\n
• Geriatric experience
\n
• Active DEA
\n
• BLS
Compensation \n
• Rates: Facility is accepting bids
\n
• Malpractice provided
\n
• Travel reimbursed
\n
