Physical Therapist

We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in private residences and senior living communities. Key Responsibilities Include Performing evaluations, developing individualized treatment plans, and delivering evidence-based therapy to support mobility, independence, and overall wellness. Documenting patient progress accurately within electronic medical records. Collaborating with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists. Educating patients, caregivers, and staff to promote safety and aging-in-place strategies. Utilizing telehealth and virtual care to enhance service accessibility and flexibility. Qualifications Include Valid Physical Therapy license or eligibility in practice state(s). Degree from an accredited Physical Therapy program. Proficiency with computer systems and EMR documentation. Current CPR certification. This role requires physical stamina for tasks such as lifting, repositioning patients, and extended walking. Clinicians will travel to patient locations, adhering to safety protocols.
