Hours 11am-7pm; 12pm-8pm

Job Summary:

We are seeking a compassionate and skilled RN Admissions Nurse to join our dedicated team of healthcare professionals at Absolut Care of Allegany in Allegany, NY . As a key member of our admissions team, you will play a vital role in ensuring a seamless transition for new residents, providing exceptional care, and promoting a warm, welcoming environment.

About the Role:

• RN Admissions Nurse is responsible for conducting thorough assessments of new residents, gathering medical history, and developing personalized care plans in collaboration with our healthcare team.

• You will coordinate with our interdisciplinary team to ensure timely and effective care, communicate with residents' families, and provide education on our services and programs.

• Your attention to detail, strong clinical skills, and excellent communication abilities will enable you to navigate complex health challenges and build trust with our residents and their families.

Responsibilities:

• Conduct initial assessments, evaluations, and documentation for new residents, including identifying needs and developing individualized care plans.

• Collaborate with the healthcare team, including physicians, therapists, and other caregivers to ensure comprehensive care.

• Communicate effectively with residents, families, and healthcare professionals to ensure a safe and supportive environment.

• Monitor and report changes in residents' conditions, documenting assessments and interventions as required.

• Participate in quality improvement initiatives to enhance the overall care experience.

• Other duties as assigned by DON or designee

Requirements:

• RN Admissions Nurse must be a licensed Registered Nurse (RN) with a current license in the state of New York.
