TITLE OF POSITION: Clinic/Medication Licensed Vocational Nurse
CATEGORY: NON EXEMPT
RESPONSIBLE TO: DIRECTOR OF CLINICAL SERVICES

GENERAL DESCRIPTION:

Provides nursing care for THE basics AT JAN WERNER under supervision of the Director of Clinical Services and in conjunction with other nursing staff.

GENERAL RESPONSIBILITIES:

• Knowledge of State requirements and operating standards and updates.
• Functions as a TEAM member to assist in the 24-hour care of Participants.
• Administration and set up of medications.
• Obtaining physicians orders as needed and daily documentation on medications and treatments.
• Occasional delivery of services, (medications, medication education, supplies, collection of lab specimens) to home bound Participants.
• Communication of Participant changes to team members.

CLINIC RESPONSIBILITIES:

• Check-in and Check-out Clinic processes and procedures for all Participants.
• Assisting Physician(s) and RN Participant Nurse Managers during Participant clinic 3-month, 6 month, and other scheduled and non-scheduled clinic visits.
• Receive and initiate Physicians orders as needed for medications and treatments. (Lab specimens, Wound Care, Ear Lavage, ABIs, etc.)
• Communicate Participant assessment findings/needs to Participant assigned RN Participant Nurse Manager (PNM).
• Maintain Clinic and Laboratory room supplies, stocking and ordering.
• Daily charting of progress notes, VS, and treatments of Participants.
• Daily charting of Caregiver face to face and telephone interactions.
• Provide routine hand and foot nail care.
• Meet other PACE documentation requirements in the medical record.
  
  

MINIMUM QUALIFICATIONS:

Possess tact, creativity, flexibility, sensitivity, a sense of humor, ability to learn and to relate to persons of various disciplines. Demonstrate basic gerontological knowledge and ability to relate to aging persons.

• Graduate of an accredited school of Vocational Nursing.
• Licensed in the state of Texas.
• Minimum of one year clinical experience in which direct patient care (geriatrics preferred) was provided.
• Evidence of leadership, creativity, integrity, initiative and communication skills.
• Knowledge of the overall organization and functions of the health care institutions in the community.
