The main focus of the position is on providing superior outpatient rehabilitation services through a collaborative care model with our physicians in order to optimize performance and enhance the quality of function for our patients./ppPhysical Therapist Responsibilities/pulliExceptional patient care/liliInterprets physician orders, performs initial evaluations and establishes treatment plans and functional goals/liliResponsible for upgrading and amending the treatment goals as necessary during the episode of care/liliWritten documentation on patient status to physicians per compliance guidelines of IBJI rehab/liliDetermine appropriate discharge timeline and appropriate follow up/liliSupervision of Rehabilitation Technicians with patient care and administrative staff as needed/liliCompliance with daily paperwork. PT/PTAs/ATCs are responsible for documenting all treatment in patient charts in a timely manner (initial evals, daily notes, progress notes, discharges, HEP)/liliStays current with billing policies and demonstrates ethical billing practices/liliMaintains active state license by earning appropriate CEUs through outside courses and IBJI sponsored educational opportunities/liliDevelops positive relationships with the physician teams at IBJI/liliTakes an active role in the patient medial team: office visits with physicians, contact with case managers, etc./liliParticipates in programs sponsored by IBJI: Mentoring, clinical educator, special interest group, community outreach/li/ulpRequirements/pulliGraduate from accredited Physical Therapy Program/liliPhysical Therapist New Grads eligible to apply/liliLicensed or eligible to sit for boards in the state of Illinois/liliAbility to work in a fast paced environment/liliProfessional and effective writing and verbal communication skills/liliAbility to lift greater than 25 pounds/liliDemonstrates good understanding of proper body mechanics for lifting, pulling, pushing/liliAbility to be stand and sit for sequences of time greater than 30 minutes/liliAbility to work on a computer for lengthy times during the day/li/ulpBase salary offers for this position may vary based on factors such as location, skills and relevant experience.
