A Community Health Worker connects people to resources in the community.

A Community Health Worker (CHW) serves as a trusted connection between individuals, healthcare providers, and community resources to improve health outcomes and reduce barriers to care.

Responsibilities may include conducting outreach, home visits, screenings, care coordination, referrals, and follow-up support while building meaningful relationships with individuals and families in the community.
