Community Health Advocate

TriHealth

TriHealth seeks a Community Health Worker to serve communities in Ohio. Under supervision, the CHW will coordinate with nurses and social workers, provide health access support, and act as a bridge to health and social services for economically disadvantaged families. The role requires basic health-related education or coursework toward CHW certification, plus BLS certification.
