Spring Creek Rehabilitation & Nursing Care Center is seeking an experienced and dedicated Wound Care RN to join our nursing team. In this full-time role, you will provide expert wound care to our residents while collaborating with a multidisciplinary team to ensure optimal healing, comfort, and quality of life.

Key Responsibilities:

• Assess residents with existing or newly developed wounds.
• Collaborate with physicians to establish and implement individualized treatment plans.
• Lead weekly wound rounds with the care team; monitor and document wound progress.
• Maintain accurate weekly wound tracking forms.
• Communicate promptly with physicians when wounds are not responding to treatment.
• Prepare and distribute weekly decubitus statistics to the wound care team.
• Conduct in-service training for staff on wound care procedures.
• Perform quarterly Quality Assurance reports to ensure compliance with wound care standards.
• Observe and evaluate dressing changes to ensure staff competency.
• Provide education on wound care to residents and families as needed.
• Participate in continuing education to stay current with best practices.

Requirements:

• Active New York State Registered Nurse (RN) license
• Wound Care Certification and/or Infection Control Certification preferred
• Minimum of 1 year of nursing experience (nursing home experience preferred)
