As a Home Health RN, You Will Be a clinical leader, delivering high-quality, personalized nursing care in patients’ homes. Craft individualized care plans that promote patient independence and well‑being. Collaborate and coordinate with a multidisciplinary team—physicians, therapists, and support staff—to ensure holistic care. Empower and educate patients and families, providing crucial teaching and emotional support. Mentor and guide LPNs, LVNs, and Home Health Aides, fostering a strong, cohesive team. Job Requirements Graduate of an accredited nursing program with an active RN license in the state served. Minimum of 1 year of nursing experience; Home Health experience required. Exceptional critical thinking, proven clinical expertise, strong case‑management abilities, and an unwavering commitment to patient-centered care. Current CPR/BLS certification. Valid driver’s license and reliable transportation.
