Offer emotional support and education to patients and families navigating end‑of‑life care. Maintain accurate and timely documentation in compliance with hospice standards. Be available 24/7 for on‑call needs, including nights, weekends, and holidays, to ensure seamless patient support. Qualifications: Active LPN license in Minnesota. Prior experience in hospice, palliative, nursing home, or home health settings is required. Strong clinical judgment paired with empathy, patience, and excellent communication skills.
