Position Description

We are seeking a dedicated Home Health Physical Therapist Assistant (PTA) to provide high-quality care to patients in various locations including Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont.

Key Details

• Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)
• Start Date: ASAP (Compliance takes approximately 2 weeks)
• Guaranteed Hours: 36 hours weekly (Note: Guaranteed hours will not apply during the initial ramp-up period of 2 weeks, holiday weeks, or time-off weeks)
• Patient Ratios: 1:1 (May visit up to 6 patients per day; 27 productivity points are standard)
• Weekends: Optional patient visits available on weekends to meet productivity requirements
• On-Call Requirements: None
• Local Candidates: Accepted

Experience Requirements

• Minimum Experience: 1 year of Home Health experience required
• Licensure: Must possess a valid California license
• Certifications: BLS certification required

Job Responsibilities

The PTA will be responsible for:

• Providing direct patient care in a home health setting
• Maintaining timely communication and documentation of patient progress
• Utilizing provided documentation tools effectively
• Transporting oneself to various patient locations, adhering to efficient scheduling to minimize travel time

Additional Information

• Travel: The traveler is expected to have personal transportation. The system will facilitate efficient client grouping to limit drive time, though some maximum distances of 50+ miles may apply.
• Dress Code: Any color scrubs are acceptable
• Parking: Free parking available

• Acute Care
• Outpatient Clinic Operations
• Rehabilitation Hospital Services
• Skilled Nursing Facility (SNF) environments
• Mobility assistance equipment (e.g., crutches, gait belts, walkers)
• Post-operative rehabilitation methodologies
• Cognitive and neurological rehabilitation care
