JOB TITLE: Supports and Services Specialist

DEPARTMENT: Business Operations Department  

REPORTS TO: Manager of Person-Centered Services

FLSA CLASSIFICATION:  Non-Exempt

JOB PURPOSE:  

The Supports and Services Specialist is responsible for leading the organization’s admissions, referral development, and transition processes. This role serves as the ambassador of the organization, ensuring appropriate placement into programs, stakeholder engagement satisfaction, and sustainable growth. The Supports and Services Specialist also assists in the development and implementation of Person-Centered Plans (PCP) by utilizing the Council on Quality and Leaderships (CQL’s) Personal Outcome Measures (POM) to identify meaningful outcomes, build individualized goals and PCP’s, and collect data to improve both service delivery and organizational outcomes. Through collaboration and commitment to person-centered excellence, this position promotes quality person-centered planning and helps to ensure services are responsive to each individuals unique goals, preferences, and needs. 

This position will be a hybrid role with some responsibility to work in the offices, in Southern Chesapeake region to include Calvert, Charles, St. Mary's and Anne Arundel area.

ESSENTIAL DUTIES AND RESPONSIBILITIES:  

 

Admissions & Intake Coordination

• Manage admissions process from referral through placement ensuring program placement reflects authorized funding, eligibility, and the needs of the individuals. 
• Facilitate timely communication between stakeholders regarding application status.
• Maintain accurate and thorough admissions documentation and tracking systems.
• Monitor admissions timelines and other performance metrics to identify and implement improvements in the admissions process while enhancing the overall experience for individuals and families served. 
• Facilitate meetings across teams, including meet and greets, facility visitations, monthly admissions meetings, and other meetings which may require in-person attendance.
• Prepare initial provider documents while collecting information from individuals served and families to facilitate a seamless transition. 
• Prepare monthly reports (i.e., AC Agenda, CQI reports), track key metrics, and identify opportunities for process improvement. 
• Provide follow-up with families and person supported post-admission 30/60/90-day check-ins to ensure fit of services and to monitor early indicators of placement instability or service gaps.
• Provide follow-up with families inquiring about services, including guidance on the DDA eligibility and application process and connecting families to additional resources where appropriate.

 

Person-Centered Planning Coordination and Improvement

• Develop and maintain PCP quality improvement system using CQL’s POM.
• Collaborate with individuals served and support team to complete POM assessments; incorporate findings into PCP goals for service planning.
• Monitor, analyze, and track personal outcome measures to identify individual and organizational trends, support quality improvement initiatives, strengthen service delivery, and advance person-centered planning practices. 
• Assist with PCP preparation by collecting and developing require documentation, coordinating meetings, and participating in planning discussions to promote collaboration 
• Perform other duties as assigned to support admissions, person-centered planning, and quality improvement initiatives.

 

EDUCATION AND EXPERIENCE:  

• Bachelor’s Degree or higher in Human Services, or other related degree required
• Minimum of three years relevant experience preferred.
• Demonstrated knowledge and understanding of Maryland Developmental Disabilities Administration (DDA) service delivery systems, and legal and regulatory requirements.
• Working knowledge Person-Centered Practices required (CQL’s POM preferred).
• Knowledge and experience with Intellectual and Developmental Disabilities (IDD) and common support strategies.
• Healthcare knowledge is preferred.

JOB KNOWLEDGE, SKILLS, AND ABILITIES:  

• Working knowledge of office applications, e.g., MS Office Suite 
• Ability to learn custom case management and reporting applications.  
• Ability to work independently and as part of a team. 
• Excellent verbal and written communication skills.  
• Ability to maintain confidentiality, tact, and discretion when dealing with personal and sensitive information.
• Excellent organization, analytical, and multi-tasking skills.
