Serenity Spring Senior Living at Northwood is a skilled nursing facility, providing clinically sophisticated sub-acute care and rehabilitation, as well as comprehensive nursing care. Coordinates medically related social services to nursing home residents overseeing the admissions and discharge process.
Duties include:
Execute admitting individuals to facility working with local referral sources and community members
Perform cognitive and psychosocial assessments and care planning
Discharge planning to include making referrals to community resources
Providing emotional support and education to residents and their family members
Advocating for residents’ rights
PHQ-9
Qualifications:
Must possess a bachelor’s degree in a human service field.
Experience working in a nursing home setting preferred.
