Setting: Home Health

Pay Range: $50.50 - $53.03 per hour

Schedule: 40 hours per week, 8am-5pm (On Call Required: Yes - Weekend on call rotation) / M-F 8am-5pm (Note: Shift includes on-call rotation)

Candidate Type: Local candidates only – travel stipend not available

Requirements:

• At least 2 years of recent Home Health experience, RN
• Qualified applicants MUST have KY RN License or compact license
• Qualified applicants MUST have experience with Wound Care, IV, Drain care, G tubes, basic education
• Preferred: HCHB
• Valid RN license in Kentucky or compact state
• Ability to analyze complex medical information and make rational decisions

Responsibilities: As a healthcare professional in a home health setting, your primary responsibilities will include:

• Perform comprehensive assessment of high-risk members (RN)
• Collaborate with primary care providers to develop and implement individualized care plans, including wound care, IV, and drain management
• Coordinate care transitions, ensuring compliance with treatment plans and addressing social determinants such as housing and transportation
