They manage medical appointments, medication administration, documentation, and follow-up while also performing all responsibilities of a Direct Support Professional in a residential setting.Primary Responsibilities- Arrange and accompany individuals to medical appointments.- Document visit outcomes, update medical logs, distribute summaries to clinical staff, and track follow-up needs.- Ensure medications are ordered, received, recorded, and administered accurately following policy.- Check medication supplies and ensure five-day supply minimum.- Maintain and file medical records according to agency policy.- Perform monthly weight checks and record results.- Receive medications from suppliers, verify accuracy, and document new medications.- Perform all duties of a Direct Support Professional including personal care, hygiene, grooming, and health monitoring.- Assist individuals at mealtime, including feeding, meal preparation, and nutritional guidance.- Maintain a clean and safe living environment through housekeeping, safety checks, and fire drills.- Support individuals in communication skills, social development, physical activities, and community integration.- Teach money management, interpersonal skills, and independent living skills.- Support community outings, transportation, and leisure activities.- Participate in program planning, individualized service planning, and behavioral documentation.- Communicate effectively with supervisors, clinical staff, families, and individuals served.- Perform other duties as assigned.Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
