Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
Deliver hands-on therapeutic interventions to improve patient mobility, strength, and functional independence
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
Educate patients and families on home exercise programs and safe mobility techniques

Required Qualifications

Active Physical Therapist (PT) license in the state of New York
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Ability to work day shift in a skilled nursing facility (SNF) environment

Preferred Experience / Skills

Prior experience working in a skilled nursing facility (SNF) setting
Familiarity with Medicare documentation and compliance requirements
Experience with geriatric patient populations
