Work Environment

The successful Psychiatric Mental Health Nurse Practitioner will operate in an onsite setting within our modern Assisted Living Facility in Rochester. Key Responsibilities

• Conduct comprehensive psychiatric evaluations to diagnose mental health disorders among residents, using standardized assessment tools and clinical interviews.
• Design and implement individualized treatment plans based on evidence-based practices, taking into account the unique circumstances of each resident.
• Prescribe and manage psychotropic medications, regularly monitoring their effectiveness and adjusting dosages based on patient needs and responses.
• Provide various therapeutic modalities such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and supportive counseling to foster emotional resilience.
• Collaborate seamlessly with a multidisciplinary team, including physicians, psychologists, social workers, and nursing staff, to coordinate comprehensive care strategies.
• Educate residents and their families regarding mental health conditions, treatment options, and essential self-care strategies to promote autonomy and understanding.
• Maintain accurate documentation of patient progress and treatment plans within electronic health records (EHR), ensuring compliance with healthcare regulations.
• Conduct follow-up appointments to reassess treatment effectiveness, manage concerns regarding medications, and adapt treatment plans as necessary.
• Participate in team meetings and clinical supervision sessions to enhance your clinical skills and collaborate on best practices within the team.

Patient Population / Client Focus

Qualifications

• Master's degree in Nursing with a specialization in Psychiatric Mental Health.
• Current and valid Nurse Practitioner license in the state of New York.
• National certification as a Psychiatric Mental Health Nurse Practitioner.
• Strong knowledge of mental health assessments, psychopharmacology, and therapeutic interventions.

Compensation and Benefits
