The specific discharge planning needs of each patient shall be identified in collaboration with the unit treatment team and may include patient interviews, record reviews, and participation in network and discharge planning meetings. Required Minimum Knowledge, Skills, and Abilities (KSAs) Education: Bachelor’s Degree in related field preferred. License/Certifications: LSW, LSW Conditional, MHRT-C, or Provisional MHRT/C required. Experience: Five years of relevant experience in healthcare required with psychiatric setting preferred.
