Full Time Registered Nurse

Full time Registered Nurse specializing in home health admissions and start-of-care visits, serving patients in Overland Park, Kansas and occasionally Missouri. The RN conducts comprehensive assessments, documents OASIS data, and collaborates with interdisciplinary teams to develop individualized care plans. This role supports quality patient outcomes and regulatory compliance in home health services.

Key Responsibilities:

• Perform home health admissions and start-of-care visits for new patients.
• Conduct comprehensive patient assessments in the home setting.
• Complete and document OASIS assessments accurately and in compliance with regulatory standards.
• Utilize Matrix EMR for patient documentation and care planning.
• Collaborate with interdisciplinary care teams to develop individualized care plans.
• Educate patients and families on treatment plans, medications, and home health services.
• Ensure all documentation meets state, federal, and agency compliance requirements.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Monitor patient conditions and communicate changes to appropriate clinical staff.
• Travel between service areas as needed, primarily in Independence with occasional coverage in Overland Park.

Qualifications:

• Active RN license required.
• Compact RN license or active licenses in Kansas and Missouri.
• Graduate of an accredited nursing program (ADN or BSN).
• Minimum 1 year of nursing experience.
• Minimum 1 year of home health or hospice experience.
• Minimum 1 year of OASIS documentation experience.
• Minimum 1 year of Matrix experience.
• Strong clinical assessment and communication skills.
• Strong knowledge of home health regulations and documentation standards.

Preferred Skills:

• Current Basic Life Support certification preferred.
• Experience performing admissions or start-of-care visits strongly preferred.
