Travelers perm address must be outside 50 miles from: 1314 Edwin Miller Blvd. Martinsburg, WV Home Health RN covering three counties: Berkeley , Jefferson, Morgan County **WVU cares needed** Cannot submit to more than 1 position at a time within WVU health system. Submission Requirements: Can only submit to 1 position at a time across the entire WVU system Drivers license address must be outside 50 miles from the facility Previous perm candidates must be gone for 2 years Previous travelers there for 1 full year must be gone for 2 years Individual unit requirements listed in pulse apply. Travel teams must be submitted together in 1 email We are only permitted to submit 2 candidates per opening, if the position is at max, we will hold on to the submission if a spot opens up Please send the information below with submittals: First, Middle, last name: Gender: DOB: Full Address: Phone Number: Email Address: On call evenings & weekends - this rotates through the staff and currently it's been an average of about 5 nights a month, which has typically been a weekend (2 days) and 3 other week nights. The on call could turn into a few different things. Some examples are: a patient is getting discharged from the hospital at 4pm and they need a nurse to meet them at home to either show them how to manage an IV, change wound care, etc. They may just end up getting phone calls to answer questions. Required Certifications WV nursing license Common diagnosis / Types of patients/patient demographics Post covid pneumonia, wound care, IV antibiotics, CHF exacerbation, copd exacerbation, post op joint replacement; mostly adult patients with occasional peds Additional Job Duties/Utilization Review involved? No additional job duties; in need of nurse to provide home health visits for general assessments, education on medications and disease processes, teaching caregivers how to provide care once home health d/c, monitoring of current illness to which home health was ordered, could also need to provide wound care, IV infusion RT Equipment (vents, Cpap, BiPap, etc) Usual RT equipment found in the home includes oxygen, nebulizer, and patients sometime already have a cpap or bipap Experience or 'Must have' skills How many patients do they see per day? Approximately 6 routine visits a day; we could discuss SOC or other oasis visits as we have not have traveler in long time. Routine visits count as 1, soc 2, recert and oasis d/c 1.5 with a goal at end of week to have approx. 30 Manager/Supervisor We have admin on call after hours and on weekends to field question and concerns. Home Health/Hospice Aids? We have home health aides during the day during the week Shifts & Scheduling Office hours are M-F 8-4:30pm and most patients are seen during this time period. We do also see patients as needed in the evening and weekends. Weekend Requirement Currently we do have a weekend nurse and so the weekend would involve being on call and seeing anyone who would unexpectedly be discharged from the hospital or would call and need an as needed visit Holiday Requirement We currently divide up the holidays evenly with the current staff; one major and one minor holiday Is there a call or standby requirement? Home health does have on call; the office is open M-F 8-4:30pm but we do need to be available to our patients after hours and so there is an on call schedule Floating Requirements No floating Scrub Color/Dress Code Royal blue is our scrub color Unit/Department specific orientation Orientation would be done by a member of our staff; we have a specific computer trainer and then the field orientation would be done by a field or charge nurse
