• Guaranteed Hours: 40

Requirements:

• Current state license as an Occupational Therapist
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance
• Two years experience as an Occupational Therapist in an acute care or rehabilitation setting
• Graduate of an accredited school of occupational therapy with a Bachelor of Science degree in Occupational Therapy

Roles & Responsibilities:

• Provide coordinated care to patients of all ages in a home care environment.
• Plan, implement, and evaluate individualized patient care plans.
• Assess patient needs and appropriateness for services, documenting progress on an ongoing basis.
• Evaluate and facilitate patient improvement in areas such as strength, mobility, and adaptive equipment use through direct therapeutic interventions.
• Collaborate with an interdisciplinary team and communicate relevant patient information while maintaining confidentiality.
• Perform clinical assessments and document patient procedures and progress in accordance with regulatory standards.
