Hospice Nurse Practitioner

Hospice Nurse Practitioner (NP)We are seeking a compassionate and experienced Hospice Nurse Practitioner (NP) to provide face-to-face patient visits and clinical oversight throughout our North Jersey service area. This role is essential in ensuring high-quality, patient-centered hospice care while supporting compliance with regulatory requirements and collaborating closely with the interdisciplinary team.Key ResponsibilitiesConduct face-to-face (F2F) hospice eligibility visits in accordance with Medicare guidelinesPerform comprehensive patient assessments to determine continued hospice eligibilityProvide oversight and support to hospice teams across Bergen, Passaic, Morris, Sussex, and Essex countiesCollaborate with physicians, RNs, social workers, and care teams to develop and update individualized care plansEnsure accurate, timely, and compliant documentation in the EMRParticipate in interdisciplinary team (IDT) meetings as neededProvide guidance and education to staff regarding hospice regulations and best practicesAssist in maintaining compliance with state, federal, and accreditation standardsSupport quality improvement initiatives and patient care outcomesQualificationsActive and unrestricted New Jersey Nurse Practitioner licenseBoard certification as a Nurse Practitioner (FNP, AGNP, or related specialty)Hospice and/or palliative care experience strongly preferredStrong knowledge of Medicare hospice regulations, including face-to-face requirementsExcellent clinical assessment and critical thinking skillsAbility to work independently and manage a regional territoryStrong communication and interpersonal skillsValid driver's license and reliable transportation (travel required throughout North Jersey)What We OfferCompetitive salaryMileage reimbursementSupportive interdisciplinary team environmentOpportunity to make a meaningful impact in patients' livesProfessional growth within a growing hospice organizationWhy Join Us?Join a team that is dedicated to providing compassionate, dignified end-of-life care.
