TheCaseManager is seeking a dedicated Case Manager to support homeless clients through intake, assessment, and coordinated care. You will collaborate with a multidisciplinary team to monitor progress and address psychosocial needs, including housing applications and referrals. The role emphasizes advocacy, client empowerment, and adherence to program policies, with a focus on individual plans and community resources.
