Home-Based RN Care Manager for CKD/ESKD

Somatus is seeking an RN Care Manager to lead VBC efforts focused on high-need CKD and ESKD patients. You will collaborate with nephrology teams, plan and coordinate in-home visits, and educate patients to improve outcomes. This field-based role requires travel within your region, strong care coordination, and excellent communication with providers and patients.
