We are seeking a passionate Case Manager to support our DRC Program in Santa Barbara, CA. As a Case Manager at our DRC programs you'll play a key role on promoting long-term, self-sustaining skills through vocational training and personal development services. Our Day Reporting Centers in Santa Barbara and Santa Maria, CA provide a community-based alternative to incarceration for clients on parole. CSI uses a curricula-driven, evidence-based model designed to assess the needs of each client to reduce individual risk factors. Participants work closely with case managers and employment specialists to develop individual goals and make positive changes in their lives.

What You'll Do:

• Perform client needs assessments within seven (7) days of intake and enter data into CSI data systems on the day the data is obtained. Enter into funder database within five (5) days of client advancement into Services.
• Actively participate in Client Progress Review (CPR) meetings by presenting new clients, assessments and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge. Provide and elicit peer support for client issues.
• Develop and document program plan goals with clients within thirty (30) days of activation and review with client on a bi-weekly basis. Enter goals and relevant client data into Individual service Case Plan and ensure ARMS database reflects case planning efforts and real time progress. Monitor program stipulations, job readiness preparation, and community resource referrals.
• Case Managers are responsible for weekly curricula facilitation including:

1. Entering all new referrals to interventions they facilitate into the CSI data systems on the day of the referral.
2. Prepare, plan and facilitate individual and group sessions with integrity and fidelity to curriculum/manual.
3. Schedule and conduct make-up sessions as needed.
4. Documentation of group and individual sessions are to be entered into the agency data systems on the same day the information is received as well as funder database.
5. Must maintain a minimum of 75% completion rate for each intervention facilitated.
6. Maintain a working knowledge of and abide by the Agency's programs, policies, procedures and code of ethics. Perform other tasks as assigned by supervisor.

Hourly Rate: $26.45 per hour

Shift Hours: Monday/Tuesday 9:00am to 5:00pm and Wednesday/Thursday 12:00pm to 8:00pm and Friday 11:00am to 7:00pm

Qualifications

What We Are Looking For:

• Bachelor's degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
• Strong decision-making, organizational, and interpersonal skills.
• Previous assessment and facilitation experience preferred.
• Experience using Microsoft Office and a commitment to working as part of a team
