Physical Therapist - Travel Contract

Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Indiana state PT license required to start the assignment. Current BLS certification from the American Heart Association or American Red Cross may be required. Experience working in skilled nursing facility or long‑term care settings preferred. Proficiency evaluating and treating geriatric patients, including fall risk assessment, transfers, gait training, and restorative nursing programs. Ability to document in electronic medical records and communicate effectively with interdisciplinary team members. Reliable professional references demonstrating previous clinical experience and patient care competency. Preferred Experience with restorative nursing programs. Knowledge of Medicare documentation standards. Experience working with rehab aides/assistants. Assignment Details Facility Type: Skilled Nursing Facility. Location: Rensselaer, Indiana. Float Requirement: Candidate may be required to float to other facilities within the region as needed. Workload: Position supports an established rehab department providing full‑time or part‑time coverage.
