Case Manager Registered Nurse (RN) Setting: Healthcare Facilities Job Type: Travel Pay: 3281 Week Hours: 40.00 Location: Hyannis, MA The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.
