Reports To – Lana Lande, Director of Community Services Work Environment – This position is primarily in-person and based at Friendly House, 1737 NW 26th Avenue, Portland, OR 97210, with frequent travel throughout Multnomah County, particularly in Northwest and Southwest Portland. Work is performed both in Friendly House facilities and in clients' homes, some of which may not be ADA accessible. Case Managers work in an ADA accessible shared office environment with cubicle workspaces. One Friendly House campus building is not ADA accessible. Schedule – Friendly House Senior Program hours are primarily Monday through Friday between 8:00 a.m. and 5:30 p.m. Occasional evening or weekend work may be required with advance notice or during critical client situations. Job Summary The Case Manager supports older adults and their loved ones in navigating the challenges of aging, including social isolation, limited financial resources, cognitive changes, behavioral health concerns, and functional impairments. Through strengths-based, person-centered, culturally responsive case management, advocacy, resource coordination, and future planning, the Case Manager helps clients access services, maintain stability, and remain safely and independently housed for as long as they choose. In addition to serving any eligible older adults in our service area, Friendly House is also a culturally specific provider for LGBTQ+ older adults in Multnomah County. Essential Functions Manage a caseload of approximately 60–75 older adult clients. Conduct intake assessments, home visits, and ongoing client evaluations. Develop and implement person-centered service plans. Make appropriate referrals to community resources and service providers including culturally specific support for LGBTQ+ clients; provide follow-up support and problem-solving as needed. Advocate for clients' needs with healthcare providers, family members, service systems, and other stakeholders. Maintain regular contact with clients to coordinate and monitor services. Document all client interactions in accordance with contract, program, and agency requirements. Spend approximately 60–75% of work time engaged in billable activities. Utilize multiple databases and data management systems accurately and efficiently. Complete reports, assessments, and documentation accurately and within required deadlines. Contribute to program development, quality improvement efforts, and agency initiatives. Attend all required meetings, trainings, and professional development activities. Support Friendly House's community-building mission by actively participating in All Staff meetings, program team meetings, and at least one Friendly House staff committee. Report suspected abuse, neglect, or exploitation of older adults and adults with disabilities to Adult Protective Services (APS) as required by law. Perform other duties as assigned. Qualifications Two years of experience working with older adults; or A degree in social work, gerontology, human services, or a related field; or An equivalent combination of education and relevant experience, especially with older adults and/or LGBTQ+ community members. Ability to successfully pass a criminal background check and motor vehicle record (MVR) check. Possession of a valid Oregon driver's license and proof of insurability required. Reliable, regular access to a personal vehicle sufficient to conduct home visits and community-based work throughout the service area, including areas not served by public transportation. Knowledge of the strengths, needs, and challenges experienced by older adults, including LGBTQ+ older adults. Familiarity with community resources and service systems serving older adults and/or LGBTQ+ community members. Understanding of professional boundaries and the ability to establish, communicate, and maintain appropriate boundaries with clients and in the community. Demonstrated initiative, critical thinking, and ability to work independently. Proficiency with computers and the ability to quickly learn new software and database systems. Excellent verbal and written communication skills, including in-person, telephone, virtual, and email communication. Working knowledge of strengths-based case management practices and person-centered service delivery. Ability to prioritize competing demands and shift between time-sensitive and ongoing projects with flexibility and professionalism. Strong problem-solving skills and the ability to work effectively in a fast-paced environment. Demonstrated reliability, punctuality, organizational skills, and effective time management. Experience working effectively with individuals from diverse social, economic, and cultural backgrounds. Preferred: Experience using ADRC CareTool, Multnomah County UCR, Oregon ACCESS, or similar data collection and case management systems. Physical Requirements Employees must be able to remain in a stationary position for extended periods of time (approximately 30–50% of the workday) and move throughout office, community, and client-home environments. The position requires the ability to operate a computer and other standard office equipment; use hands and fingers to handle objects and equipment; communicate effectively; and exchange accurate information. Employees may occasionally climb stairs, walk on uneven surfaces, stoop, kneel, crouch, bend, or reach. The ability to observe details at close range is required. Employees may occasionally lift or move items weighing up to 30 pounds. Work may involve exposure to outdoor weather conditions, temperature variations, household odors, and other environmental conditions encountered in community-based settings.
