RN Case Manager - Home Health Location: Covington, Tennessee Company: Strong Force Staffing (RPO for Client Agency) Overview Welcome to Strong Force Staffing - Building Stronger Teams for a Stronger Tomorrow. Responsibilities

Conduct comprehensive patient assessments.
Develop and oversee individualized care plans.
Coordinate care with interdisciplinary teams.
Educate patients and families on disease management.
Monitor patient progress and adjust care plans as needed.

Requirements

Certification: Licensed Registered Nurse with no restrictions.
Experience: Minimum 2 years in home health care.
Skills: Excellent clinical and communication skills. Strong organizational abilities. Proficiency in EMR systems.
